Contraceptive Services: Patients outside of the practice may self-refer by calling 719-285-8890 or other providers may refer their patients by fax 719-789-8074, for a limited number of visits to consult on device selection, insertion and/or removal of devices, and for troubleshooting after device placement.

Dr. Chollet is a traditional fee-for-service provider. Please note that "problem-based" discussions during a "preventive exam" may result in a separate co-pay per insurance guidelines.
