Dr. Chollet is a board-certified Family Physician with an additional fellowship training in obstetrics. She provides compassionate care to individuals and families in all ages and stages.

We are currently scheduling patients previously under Dr. Chollet's care. With the exception of prenatal patients, new patients will not be considered until January 2026.

Insurance Accepted

Dr. Chollet accepts most major insurance plans. At this time, some of our insurance contracts are still pending. Please call our office to speak with Tara regarding specific inquiries.

Getting Scheduled

Online scheduling is not yet available, as we are currently in the process of registering our patient panel. In the future, online scheduling will be available via the Elation Passport.
